Your enamel is not a smooth shell. Under a microscope it looks like stacked rods — hexagonal crystals of a single mineral, packed tight and bound by protein. That mineral is hydroxyapatite. Roughly ninety-seven percent of your enamel, by weight, is already the thing most toothpastes now claim to "restore."
The question is not whether hydroxyapatite belongs in a tube. It is whether the version in your tube can actually reach the places enamel loses first — and at a concentration that does something measurable.
What the crystal is doing
Hydroxyapatite is calcium phosphate in a specific crystal form: Ca₁₀(PO₄)₆(OH)₂. In enamel it sits in organized bundles called prisms. When acids from food or bacteria pull minerals out, those bundles lose integrity at the surface first. Sensitivity, chalky spots, and early white lesions all start at the same place: microscopic gaps where crystal is missing.
Remineralization is the reverse process — new crystal filling those gaps. Fluoride works by forming fluorapatite, a harder variant. Hydroxyapatite works by depositing more of what was already there.

Nano vs. micro — and why both
Particle size determines where hydroxyapatite can go. Micro-hydroxyapatite particles are large enough to coat the enamel surface and fill visible micro-lesions. Nano-hydroxyapatite particles are small enough to enter open dentinal tubules — the channels that connect the tooth surface to the nerve.
Two scales, one mechanism

Most brands pick one size and call it innovation. Clinical work from Japan — where hydroxyapatite has been a recognized anticaries agent since 1993 — supports using both simultaneously at meaningful concentration.
Why ten percent matters
Concentration is the part marketers prefer to omit. An 18-month double-blind trial on adults found that 89.3% of participants using hydroxyapatite toothpaste had no increase in their cavity index — but those studies used formulas at or above ten percent active HAp. Many retail tubes sit at one to five percent and lean on the ingredient name alone.
